How do I enroll in the vision plan?
To enroll for coverage under the plan, you must complete and sign a vision
enrollment form (your coverage will be effective on the first day of the following month after GEBA receives your enrollment form). Once completed, you can submit your enrollment form to GEBA in any of the following ways:
Fax: Fax to (301) 688-6694 (no cover sheet needed).
Email: Scan your completed form and email to
geba@geba.com. Include “Vision Enrollment” in the subject line.
Mail: Mail the completed form to GEBA: GEBA, Inc., P.O. Box 206, Annapolis Junction, MD 20701-0206.
Remember, because your benefits renew annually, participants must commit to one full year of premium payments upon enrollment.
How can I pay for my coverage?
Premiums can be paid bi-weekly through payroll allotment (NSA and DIA employees only), automatic debit (monthly, quarterly, semi-annually and annually) from a checking or savings account, or mailed paper billing (quarterly, semi-annually or annually). Keep in mind that a $2.00 service charge will be applied to each bill you receive in the mail when using GEBA’s quarterly and semi-annual direct (paper) billing service.
How can I use my contact lens services?
For more information on the mail order contact lens replacement service, you may access Contact Fill’s services online at www.contactfill.com or by calling them toll-free at (866) 234-1393. Just mention you have NVA vision insurnace through GEBA, give your name and/or the alternate ID number found on the FRONT of your insurance card.
After enrolling, how do I print insurance cards from the NVA website?
In the event that would like to use your services before you receive your official insurance cards from NVA in the mail, you may print your insurance cards* from the NVA website at www.e-nva.com.
To print ID cards from the NVA website:
- Visit www.e-nva.com and click on “Subscriber Registration” on the right side of the homepage.
- On the following page, complete the required information. In the “Subscriber ID” field, enter your GEBA member ID with three zeros in front (000123456). Call the GEBA office at (301) 688-7912 or (800) 826-1126 to obtain your member ID.
- If you're already a GEBA member, you may call at anytime to obtain your member ID.
- If you're NEW to GEBA, please wait 48 hours after you've submitted your enrollment form to GEBA to call for your member ID to allow for processing time.
- Once logged in, the option to “Print ID Cards” will appear on the left side of the screen as the second option.
What if my current eyecare provider is NOT in the NVA network?
If your current eyecare provider is not in the NVA network, they can be! Just let your provider know they can register to become a provider on NVA's website. Once the quick-and-easy form is completed, a NVA representative will contact them to discuss the details. It's that easy!
And, remember, even if you use your insurance with your provider (that is not in the network), you can still take advantage of the many reimbursement options available for your services. For example, you can get up to a $40 reimbursement on your annual eye exam or even up to a $130 for contacts in lieu of lenses.

*Keep in mind that your insurance cards will be mailed to you within 3-5 business days after your coverage effective date. If you would like to print your insurance cards from the NVA website, you may print these cards 48 hours AFTER your Vision Insurance Enrollment Form has been received in the GEBA office.
