Claim Forms must be obtained directly from VBA either by calling 1-800-432-4966 or from the VBA Web site. When prompted for your social security number, please remember to use 000 your member ID number. Please contact the GEBA office for your member ID number.

Frequently Asked Questions

  • What are the benefits provided by the plan?
    Plan members are eligible for a covered vision exam and new lenses every 12 months and new frames every 24 months. If contact lenses are selected in lieu of glasses, the member will be reimbursed for a total of $130.00 (to be used towards contact lenses and vision exam).
  • How much does the plan cost?
    For employee coverage only, the cost is $104.00 per year. To cover an employee and spouse, the cost is $182.00 per year, and for a family plan, the cost is $260.00 per year.
  • What are the benefits of using a VBA Participating Provider?
    Using a VBA Participating provider provides 100% cost coverage of a vision exam, lenses (clear single vision lenses, bifocal, trifocal, lenticular, solid or gradient lens tints, UV 400 coating and scratch resistant coatings), and frame, up to plan maximum.
  • What if I wish to use a provider that does not participate with VBA?
    Under the plan, you can use any provider you wish. You will still be required to request a validated VBA benefit claim form. After you have received your desired services or products, simply attach your receipts to your VBA claim form and submit for reimbursement according to the scheduled benefits. You may wish to use a non-participating provider for your exam and a participating provider for your lenses and frames.
  • Who are the VBA Participating Providers in my area?
    An up-to-date list of providers in your area is available on the Vision Benefits of America Web site. Brochures are also available in the GEBA office, or a list will be sent to your home along with a validated benefit claim form when you request services from VBA.
