The premiums for long term disability insurance are based on your age, monthly benefit and waiting period. The total amount of disability benefit can be up to 67% of your annual gross salary.

- Survivor Income Benefits, when available, are paid to the insured’s designated beneficiary (a benefit equal to three monthly disability payments).
